I'm using Firefox 86.0 with NoScript 11.2.3 on Windows 10. The NoScript icon in my toolbar permanently shows the icon of the "s" with "?" graphic. If I click on the icon, no menu appears even after repeated clicks. I can right click on the icon to manage, remove, report, etc. Right clicking for the context menu on any webpage does not show any NoScript options. I've gone into Manage Extensions have tried disabling/re-enabling and uninstalling/reinstalling NoScript to no avail. I tried re-installing Firefox and again same result.

It seems to me that NoScript isn't running at all. I'm getting ads on websites that I normally don't see. This started happening a couple of weeks ago; I'm uncertain what version of NoScript was installed when that happened.

Could you please open about:debugging > This Firefox and click the "Inspect" button in the NoScript section?

The console is constantly filling up with errors such as these:

[NoScript] undefined Error: An unexpected error occurred An unexpected error occurred <empty string> log.js:17:13

(there are more to the messages but having a hard time posting through the forum's spam filter)

Try completely quitting Firefox and restarting it with -purgecaches command-line option. If that does not help, does this problem occur in a new, clean Firefox profile with only NoScript installed and no changes to any settings?

Command line restart with -purgecaches did not work. This problem does not occur in a a clean profile with only NoScript installed. It worked fine like that.

In my normal profile, I tried uninstalling all add-ons I have (NoScript, Adblock Plus, and DownThemAll) and then reinstalling Noscript by itself, but NoScript still did not function properly.

This suggests your profile is corrupted.
Please try migrating your data to a new profile using Firefox Sync.

I ended up manually moving a bare minimum of my old profile data into a new profile. The issue followed the new profile if the original prefs.js file was used so I simply ditched it. Everything is working fine now.

Thanks a lot for your help! I've been using NoScript for quite some time and I'm pretty happy with it!

I've this issue too.
Solved by (where FF profile resided).
After hdd error correction, NoScript works again.
FF 91.7.0esr
NoScript 11.4.3

Maybe it will be useful for someone
