Problems with NoScript menu

Hi, I've had NoScript installed for a while, working nicely. However I noticed recently that the menu I see from clicking the icon doesn't show the same list of menu items it once did. Specifically, it doesn't show 'Temporarily allow ..', while it does show the list of 'Mark .. untrusted' which I think used to be in a sub-menu. Let's call the default menu the 'GoodMenu' and my messed up menu 'BadMenu'.
What I have done so far (with, and without, browser restarts between every change):
1. Export my settings in Options, then clicked 'Reset'. Result: the menu items revert to GoodMenu. Good.
Import my settings and whitelist again. Result: BadMenu. Bad.
Conclusion: there's probably no bad interactions between NoScript and my browser config or other extensions/add-ons, since NoScript is fine after clicking 'Reset'. The problem lies with my NoScript config.
2. I carefully ensure that the 'Appearance' options are identical both with my own settings and with the default settings I get after clicking 'Reset'. I still get BadMenu.
Conclusion: either there is another option that affects the appearance, that I have missed, or that the appearance settings are somehow not being enforced.
3. I installed the latest development version: no effect on any of the outcomes described above.
4. I deleted the extensions.* files in the profile folder. Re-enabled all the extensions. Result: Still get BadMenu and what's more, some of my extensions (including NoScript and Privacy Badger) are now marked with the warning: " XX could not be verified for use in Firefox. Proceed with caution" (a signing issue, I believe).
If I scratch my head much more I'll go completely bald. Any suggestions most appreciated.
(My setup: Win 8.1; Firefox 45.0.1; Privacy Badger, Disconnect, uBlock Origin, HTTPS Everywhere, Self-Destructing Cookies, NoScript; Tabs on bottom, FB Purity, Sage (RSS reader), NewTab homepage. Everything kept up-to-date.)
