Where are Flash permissions stored in NS?
I want to review all sites where I have allowed flash to run without asking.

Can be stored in about:config > noscript.allowedMimeRegExp
and (if you have not checked "Apply these restrictions to whitelisted sites too") the normal NoScript whitelist

My noscript.allowedMimeRegExp says default.
I don't see where "Apply these restrictions to whitelisted sites too" is in the NS Options.

Yes, I do have "Apply these restrictions to whitelisted sites too" checked.
I still do not see a place in NS where I can view where I have and have not allowed flash to run without asking me.

So if you selected to forbid Flash, you can review where you have allowed Flash to run automatically by looking at allowedMimeRegExp (which is empty in your case, so you should have no sites that run Flash without your explicit permission).

I believe what you are looking for is not in NoScript, but in FF itself.
If you left-click a sites "icon" in the URL bar, More Information | Permissions, you will find the Flash (& other) permissions for that site.

Also, I know for a fact that there are site where I have allowed flash to run without asking me, so I am not sure why that setting says default.

I don't have time to left-click each site's "icon" in the URL bar.
I was assuming that NS kept a record - somewhere - of what sites I had allowed to run flash, since it is done through NS in the first place.

How are you Allowing these Flash through NS?
