I notice your menu is missing "Allow [site]" commands as well

Is that Firefox window in Private Browsing?

No its just unchecked in options, as in the image, if I check the box for it it comes back in the menu.
Make page permissions permanent does not appear whether the box is checked or not.

I did revert the settings to default, uninstall noscript then reinstall noscript.
I'm left with no sites permanently allowed & having to temporarily allow every site I visit now.

I guess uninstalling firefox & deleting the profile folders in app data is the way to go fo me now.

I have
1. uninstalled firefox
2. deleted appdata\Roaming\Mozilla\Firefox
3. deleted"%userprofile%\AppData\Local\Mozilla\Firefox
4. installed firefox
5. installed noscript

It is now back to normal.

I see now what you're referring to. Well all is back to normal now.
Just to clarify it wasn't a private window.

Seems I was in a private browsing window as I had the following highlighted checkbox checked

Could we have the make page permissions permanent option for private windows?

Yes, two ways to do that:

1) GUI method
NoScript Options > Appearance, check "Permanent 'Allow' commands in private windows", then check that menu item you now see in NS menu in private window
(Not sure why Giorgio thought it best to remove that option itself from Appearance when requested that the menu item be customizable through Appearance... [ETA viewtopic.php?p=73860#p73860 ])

2) about:config > set noscript.volatilePrivatePermissions to false

Bear in mind though that this feature was added because if you're whitelisting sites while in Private Browsing, you're saving data about your browsing habits while in Private Browsing...
