Allow top-level temporarily standard options not working
For some weeks now when I open new sites in FF i always get the "Java script forbidden" bar although the option to allow top-level domains temporarily is on. It does not matter which of the three sub-options is chosen. Java-script gets enabled by hitting f5.

Just to be clear, if you go to http://isjavascriptenabled.com/, the first time you go there it says no but if you reload it says yes? Or is the notification just wrong?
Does the issue occur in a clean profile with only NS and all defaults (except for temp-allow top-level sites)?
What if you then completely shut down Fx, and copy only your prefs.js from your main profile to the clean profile?

(1) The answer is yes
(2) The answer is no, then it works ok
(3) I have an almost clean testprofile. It works ok, also when I copy the prefs js to it.
(4) Turned batches of other extensions off, then on. Problem persists (list of extensions added)
(5) Curious thing is it even blocks sites initially I have explicitly added to the whitelist.
(Added) I didn't notice before: I have two independent profiles of FF running at the same time with the same extensions active. The other profile doesn't suffer from the problem. The tests above I did with the second instance closed.

These things are usually something to do with some pref(s), so try this:
Completely shut down all Fx, backup both profiles, then try copying the prefs.js from the working profile to the not working profile. If that gets it working, do a diff of the working prefs.js and the "bad" prefs.js, and try to isolate the culprit pref(s) from there.
If that doesn't work, see if you can narrow down which file(s) in your profile is causing this? (completely shut down Fx, then remove some files from your profile, see if the problem is gone, if so, put some back, repeat)

Since my experimenting the problem has become less. I just compared the two prefs and, other then differences in nosrcipt settings, I don't think there are any that could cause the problem. I have synchronized the settings, most notably turning off(!) "Cascade documents't permissions..."
