NS breaks Fx's per page plugin permissions dialog

Fx 24.0.0, NS 2.6.7.1, new profile, block flash on trusted
- https://github.com/mozilla/mozilla-central
- allow github.com, fastly.net; reload
- click on the plugin permissions button (lego piece icon on the left of the urlbar)
- the dialog is messed up, extra button, missing text

Reproducible on FF 24 / Pale Moon 24.

this has regressed at some point after the fix, present in Fx 24 & NS 2.6.8.5rc1
