Steps I have taken:

1) Uninstall and reinstall No Script.
2) firefox.exe -p New profile.
3) Turn off all plugins uninstall and reinstall.
4) Customize toolbar. (No Script Icon is not in the list)
5) New profile no add-ons remove all plugins install.

The icon in the navigation toolbar is gone. The icons on the page (in place of JavaScript code) is missing. The info bar at the bottom is gone. I get no warning and I verified the options selected.

No Script is working, all the java script websites are blocked. I can manually add whitelist in the options menu under tools add-ons.

The only way to resolve this was to reinstall FF from Internet explorer. I had tried to reinstall from within Firefox and that did not work. From IE I downloaded the Executable and then ran it. This gave me all my NoScript icons back.

All my plugins are also activated.

In the FF Customize Toolbar dialog, you might try 'Restore Default Set' & see if that helps?
