Steps to reproduce:
1. Click on http://people.mozilla.com/~nhirata/html ... _480p.webm
Actual results:
NoScript shows a placeholder at the bottom right of the browser window
Expected results:
The placeholder should be shown in the centre of the window
Tested using Firefox 18 beta and NoScript 2.6.4.2rc3.

I see this too.
My whitelist does NOT have "mozilla.com".

Temporarily allow
This 'place holder' was on the 'top left' of the 'big place holder', which was as
mjh563 said is at the bottom right of the browser window. So, easy to 'see'.
Also,
if you right click the video you can 'play / pause'.
The "Hide controls" / "Show controls" don't show.
From MY limited point of view (don't play video much) not a big deal.

you have to put the URLs in BB 'square brackets' like this
