Just wanted to document what fixed my issues after the 10.1.3c3 update.
1. Uninstall NoScript 10.1.3c3 / Shut Down Firefox
2. Start Firefox / Install NoScript 10.1.3c3
3. Open the NoScript Settings
4. Check a few a websites listed as "Trusted". You will see that only "Script" is checked.
5. In one of these "Trusted" websites, select all the other boxes.
6. Now verify that all websites listed as "Trusted" have the boxes checked.
7. Close the NoScript Settings
8. Problem should now be fixed
(Optional) 9. Open the NoScript Settings and verify that all websites listed as trusted still have the other boxes selected.
Note: Re-install was required in my case. Without doing a reinstall, at #6 the "Trusted" websites did not have the boxes checked (ie it appears the default settings for trusted websites were not being saved). It was not clear from the other suggested fixes that an uninstall and re-install was required and there are a lot of people who seem to be having the same issues, so hopefully this is helpful to everyone.

This did not fix any of my issues. It got worse.
All sites are gone (maybe that is supposed to be like that?).
Now I cannot add sites to trusted in preferences as well (entry box at the top).
I don't see the noscript icon anymore and cannot add it. It is not available in customize firefox.
