I have encountered an unusual problem. I pick and choose which websites I allow scripts to run on. I have found in several cases that, apparently, sites that I allowed scripts to run on had turned from this setting to S! or global allowance of scripts on its own. Is it possible for this to happen or could I be subject to a worm/virus. I protect my machine with the McAfee security suite as well as by several spam blockers.

No web content can change NoScript's settings (quite obviously, since it would completely defeat its purpose).
Unless you used "Allow scripts globally" by accident (which you seem to exclude), most likely culprit is another extension interfering.
Should you be able to reproduce the issue, I would suggest Standard Diagnostic.

I am not sure if this is your issue in particular, but I have encountered from a couple friends that when they have the setting "Left clicking on NoScript toolbar button toggles permissions for current top-level site (ctrl+shift+BACK SLASH)" on the General tab of the NS settings checked, they will occasionally notice that either accidentally or via bad clicking, they end up allowing a site. For most of them unchecking that has been the solution. I hope that's your issue if not, then I am sorry.

Well Giorgio beat me to it and I would do what he suggests, I was already responding when he posted and didn't see it.
