It's quite complicated, involving threads and event queues.
Oversimplifying, MGR got called too early, when the request it was observing was not initiated yet.

I am glad it helped you and certainly I'd be happy to tell you. The AMO archives can be easily reached by going to the NoScript addon page at https://addons.mozilla.org/en-US/firefox/addon/722 (can also be reached by simply going to the addons.mozilla.org and searching for "NoScript") and then if you scroll down a bit towards the bottom of the description before the reviews, you will see a link to the latest version, if you click on that it will take you to the archives with the most recent highlighted on top but you scroll down on that page, you will see all the previous versions as well, the link I gave your for AMO just shows you everything ever submitted to Mozilla with most recent to oldest.

And as for the NoScript archive provided kindly and organized with the highest fidelity by Giorgio, you simply need to go to http://www.noscript.net and the simplest way to get the released versions new and old is to click the RSS icon in the URL bar () which loads this http://noscript.net/feed or bookmark it for quick reference in the future. Now to get the dev builds, the ones still being worked on, redrafted, tested and not formally released until they are ready, you can just go to http://noscript.net/getit which you can reach from the noscript page on the top menu, via direct bookmark or even via a link provided in the board index of this forum. To get ALL of the released AND dev builds in one shot you can just go to this special feed http://noscript.net/feed?t=a kindly provided by Giorgio for easy access and you can bookmark it. Hope that has sufficiently covered how the URLs provided to you were arrived at.
