It has been several months this is problem occur. Even though Firefox, Flash Player, and Flashgot is always updated to the latest.
Current installation:
- Firefox 18.0.2
- Flash Player 11.6.602.168
- Flashgot 1.5.4.1 (the only enabled extension)

its working again, after complete reinstall windows due to hard disk failure
I can't found the exact problem for this case either,
Hence for anybody who face the same problem, try to reinstall Firefox, Flash Player
