The problem is very elusive: it doesn't happen if you install the add-on and restart immediately, but it happens on cold start, as it was related to XUL cache.
I've made several test but couldn't figure out the root cause yet, let alone a fix...
If it helps:
Any new tab will NOT have the bar, but the bar persists with the original tab. Closing then reopening the tab to the same page also clears the bar, as does changing the visibility of ANY toolbar (not just the addon bar).

I can confirm I am also getting this issue with Firefox 12. The issue does not crop up when I restore a previous session.

I can also confirm the issue does not show up if you have the menu bar enabled

You guys are right about when "Menu Bar" is enabled. It does not show this problem. I hope this is a clue or can help troubleshooting the issue for the dev. Any progress yet?

I also have this problem. If there is anything I can help with let me know.

I'm using FF 12, with the menu bar and add on bar disabled.

If I press ctrl / two times it will clear the white bar.

Saw the recent version update - still no closer to resolving this it would seem? :cry:

In facts, the bug is very elusive. I even removed the whole NoScript overlay (the UI elements and modifications), but it keeps happening.
Still investigating...
