If this is exactly the same problem, you should have a NoScript submenu in the context menu, when you right click on any page.

Did you try disabling every add-on (especially themes)? If that works, please try Standard Diagnostic to find the evil add-on (in the OP case, it was the mozilla-kde integration).

I can confirm the problem on openSUSE. For me, the offending package was mozilla-kde4-integration 0.6.2-1.9

After removing the package and starting FF again, the icon showed up as it should.
