As above, STR:
1) FF 45.5 ESR new, clean profile
2) Install NoScript. In Options/Embeddings select "Forbid <FRAME>" and "Forbid <IFRAME>", and "Show placeholder icon".
3) Go to http://www.liveleak.com/view?i=474_1479906332
The video is actually embedded from youtube.
The expected result is to get a placeholder, works as expected in FF 50.

(On Youtube itself) I'm not seeing a placeholder at all, so nothing to click, so no way to allow the object.
Actually I can't get any (media) object to show on Youtube at all with NoScript enabled.
