Simple PayPal Integrated Forms

Hello all, I was wondering if I could pick your brains for some suggestions. I have done pro API integration for PayPal for many reason for a while now and have no personal trouble with it; however, I come across people who ask me for a tool that will generate simple payment pages that are not based on the button system.

Let me elaborate a bit on what they are looking for, something to this effect:

Some title
Some content
Some instruction
bluh bluh
Enter payment amount: <textbox>
Enter credit card number: <textbox>
Enter expiration: <month list>/<year list>
Enter CCV: <textbox>

If paying an invoice, enter the reference number...bluh bluh <textbox>
<pay button>

and it will take that amount, use the hidden field for the receiving paypal account and submits that amount, using that card, and viola, done. We are talking about supremely basic here and if I had the time, believe me I would just write a utility for this and probably make a bundle selling it for $5 a piece, I have seen some crappy utilities making serious money on stuff like this.

Anyway, I want to know if anyone of you knows a generator/utility that creates pages like this for the user based on their credentials so they can just pop it on their website and viola be done? This would cut so many requests out of my mailbox and would be greatly appreciated. Ideas?
