I just had an instance where the Allow Global icon only displayed momentarily before reverting back to the Scripts Partially Allowed icon.

Restarted my browser (SeaMonkey 1.1.17), & now I cannot duplicate.

Is it actually a "text" file, or is it that the saved download happens to have a ".txt" extension?
If it is a text file, what does it say?

What is DTM?
If instead of using DTM, you use Browser Built In, do you have the same problem?

You are strongly encouraged to upgrade manually to latest stable version (1.9.3.3).

1. Not seeing that. Also don't see file: listed in \defaults\preferences\noscript.js. OK, I misread. file:// is not appearing in the NoScript context menu, but it is being added to the whitelist (Options | Whitelist). Further, when Forbid a site or do a Reset, it again ads file:// to the whitelist....
