I figured it out. The icon was missing in the customize toolbar dialogue box. Upon closer inspection, the text was there indicating where the icon was supposed to be. Clicking in the blank spot under the text added the icon to the add-ons toolbar.

Using flashgot 1.4.6 and firefox 13.0.1 and the blinking icon does not show up on flash video pages anymore. I can put the static icon on the add-ons toolbar at the bottom of firefox and I can still right click the page for to the tools menu and click flashgot media but either option still downloads...
