Very sorry to jump in on such an old thread, but I was having the exact same problem described by the OP. (NSA would stop working after firefox was closed; reinstalling NSA fixed it until firefox closed again.) After several weeks of reinstalling NSA, I eventually tried setting FF's browser.sessions...
