The issue with doing that is that it will also throw out my 2 Factor Authentication code cookie which I have told it to trust a particular computer. The result is that every time I log into my Google accounts on that machine I have to enter the 2FA code plus username and password.

Same thing happening here. Rollback to 2.9.0.14 fixed that issue for now

This is the screen shot of the phantom noscript that remained and could not be deleted. I noticed that in my Firefox 45.0.1 x64 had two entries in the extension list. One showed that Noscript was to be installed and the other showed that NoScript was suppose to be uninstalled on the next reboot. Rebooting solved nothing they still showed up in the extension list. Well I have the following whitelisted:

dslreports.com
dslr.net
cloudflare.com

and I still can not get the speedtest to work. I still see their pop up showing up. I can not whitelist the subdomains you indicated since the main domains are already whitelists

Hmm. That would certainly explain why lastpass.com is not showing up on the menu. I would certainly recommend disabling cascaded permissions if you can. I have lastpass.com in the whitelist so under Untrusted there is the option to Mark Lastpass.com as untrusted and that is it. I found that the Account Settings link works if I uncheck "Cacasde top document's permissions to third party scripts" under the Trusted tab. I believe that needed to be checked on for another website to work properly.
