All right, I figured it out. Turns out it is a combination of NoScript + Greasemonkey + the Greasemonkey script "YouTube Enhancer." Remove any of these from the equation, and the bug doesn't happen. My best guess is it is the "YouTube Enhancer" script, so consider this bug report...
