CCleaner Free v5.69.7865 (64-bit)
Custom Clean: Applications: Firefox: Internet Cache
Run after closing FF
Run while Enabled: Permanent permissions for incognito changed to unchecked; Per-site-permissions reverts to default
Run while Disabled: Permanent permissions for incognito remains checked; Per-site-permissions changes remain

CCleaner does seem to be the culprit. Specifically, it appears to be the Internet Cache option (under Custom Clean -> Applications -> Firefox).

Looks OK to me (Nightly).
Might you have another extension that is interfering?

Is it only newly set permissions that are not keeping, or have older set permissions also disappeared?
